DELTA PUBLIC LIBRARY REGISTRATION POLICY
NEW REGISTRATIONS: Adult patron must furnish a picture identification such as a driver’s license or state ID and some form of current address verification, whether applying for self or minor child. (Same ID card may serve for both purposes.)
ALL patrons are REQUIRED to supply the following information:

A parent or guardian must sign the registration card for students ages 5-18. Since this is a contract with a minor, signing the juvenile card means the parent or guardian will be held responsible for any library material or fines checked out on the juveniles card.
CARD USE:
Cardholder must be present to check out material on his/her card. Library cards may be kept at the library on file if patron chooses. Library card holders are responsible for all material checked out on their card. Library cards are not transferable. Patrons may have only one library card number at a time.NEW CARD HOLDERS:
New borrowers may only take 4 items at a time for the first three months.LOST PATRON CARD: A lost card should be reported immediately. After a one week waiting period (FROM THE TIME THE CARD IS ACTUALLY REPORTED LOST), if the card has not been found, there will be a charge of $1.00 for the first replacement card, $2.00 for the second card, $3.00 for the third, and so on.
Once a new card has been issued as a replacement, your old card will be marked VOID. If your old card is found, please destroy it or return it to the library. A patron will not be allowed to check material out on a card which has previously been reported lost.
STOLEN CARDS: Will be replaced free of charge if a copy of the police report is furnished by the patron.
CHANGE OF LAST NAME OR ADDRESS:
Notification of change of name or address is the responsibility of the cardholder.
